A
functional assessment and income-based (125% of HHS Poverty Guidelines) service
fee of $10.00 are used to insure the targeting of frail, elderly clients with
the greatest socio-economic need.
MEMORIALS
Memorial
contributions are accepted. Unless
otherwise designated they will be used for direct client services.
REQUESTING
SERVICE
Contact
our In-Home Services Coordinator at (231) 839-7839 or toll free at
1-888-616-3102. Our Coordinator will
schedule an appointment for an in-home assessment and determine eligibility
based on the outcome. Referrals are
accepted from older adults, family members, physicians, hospital discharge
planners, and concerned individuals.
IN-HOME
SERVICES
Programs
include personal care, respite, homemaker, and home chore services.
These services are available after potential clients have been assessed
by our In-Home Coordinator and are found to be in need of assistance in
maintaining a safe, independent lifestyle. Medication
management and foot care are also available.
These skilled services are
FAMILY
CAREGIVER SUPPORT GROUP
This
group meets the 4th. Thursday of the month at the COA office in
HOME
CHORE PROGRAM
This
program includes lawn care, snow removal, deep cleaning, and minor home repair.
Some portions of this program are income based and may have a cost-share.
Call the office for additional information.
Transportation
Assistance
Persons
60 and over, or under 60 with a permanent handicap, can request assistance.
A 48 hour notice is required in order to schedule a volunteer driver.
The program, funded by a MDOT Grant, is intended for non-emergent medical
transportation and essential trips.
The
Commission on Aging works in cooperation with the following local center.
