ADULT
PROTECTIVE SERVICES

Adult Protective Services
This program endeavors to protect seniors who are in danger of harming
themselves or others or who are being exploited by someone.
The difference between child protective services and adult protective
services emphasizes that adults are allowed to make poor choices and as a
result, sometimes remain in difficult situations.
The
Department of Human Services (Family Independence Agency) is located at 448 Court
Plaza, Beulah.

Northwest Michigan Elder Abuse Awareness Committee
This committee develops, strengthens, and administers programs that
create awareness for the prevention and treatment of elder abuse, neglect and
exploitation in the ten counties of Northwest Michigan.
They conduct presentations
throughout the region, collaborate with county professionals monthly, and
provide resource materials. Northwest
Michigan Elder Abuse Awareness Committee is associated with Catholic Human Services
and located at 3210-B Racquet Club Drive, Traverse City.

ASSISTED
LIVING FACILITIES
· Frankfort Pines A twenty unit Assisted Living Facility offering studio and one-bedroom apartments and suites for single residents and couples. Services provided in the monthly care rate include: Assistance with Dressing, Bathing, and Grooming, 24 Hour Awake Staff, Three Meals a Day plus Snacks, Utilities (except cable and telephone), Internet Access, Housekeeping, Individual Laundry Service, Medication Management & Ordering, Health Care Coordination, Planned Activities and Health and Wellness checks. Frankfort
Senior Care Home
Provides assisted living in a clean, safe, and caring environment.
There are ground level, private apartments in a ranch home and just four
blocks from hospitals, doctors and shopping.
The monthly rate includes 24-hour assistance with personal care, a
24-hour emergency call system, supervised medication management, home cooked
meals and special diets, laundry and housekeeping services. Also available are short term care, day care, and respite

Green Acres This is a
beautifully landscaped retirement living center offering 20 one-bedroom
apartments. Included in the private
pay monthly rent is weekly housekeeping, laundry services, medication
supervision, and 3 home cooked meals each day.
Staff is on duty 24 hours every day, there is an emergency call and
security system, activity area, assistance with medical and personal needs,
beauty and barbershop, central air conditioning, and heated sidewalks. Green Acres is located near restaurants, shopping

The Homesteader
This twenty-two unit service-oriented senior housing facility offers
one-bedroom apartments for residents. Services provided in the monthly rate
include: maintenance, utilities, housekeeping, laundry, three
“home-cooked” meals daily, care management, respite care, 24 Hour
emergency call system, transportation and networking with outside agencies.
Pets are welcome at no extra charge. The Homesteader is located just one mile
from shopping at 7953 Love Road, Benzonia.

Maplelawn Assisted Living
This spacious home is located in Benzie County between Honor and
Beulah. The home is set up with
all ground floor services. We
provide 24-hour supervision and assistance with all Activities of Daily
Living (bathing, grooming, dressing). Services
also include: care management,
medication management, housekeeping, laundry, home cooked meals, and
networking with outside agencies. We
provide individualized care and attention to each of our residents.
Our residents participate daily in meaningful activities, therapies,
and all aspects of daily life. This
home is equipped for seven seniors.

Orchard
House
This is a unique assisted living facility in a
peaceful and beautiful setting. Our units are located in Benzie County on Grace
Road five miles from either Frankfort or Benzonia. There are six single bedroom apartments and one short stay
room. Care is individualized and directly managed by a Registered Nurse and
owner, Jeryl Colby. Meals are home cooked and provided in a new community room.
Apartment space is also available for visiting family members. Our philosophy
and mission is to treat your family like they were our own family.

Caregiver Resource Center Library This
library has an assortment of over 350 general health and caregiver related
resources including videos, books, journals and magazines.
It has two computers that are set up with user-friendly search engines
and websites to assist individuals with finding reliable health information on
the Internet. The library is open
to the public every day, from 8 a.m. to 8 p.m.
Volunteers staff the library intermittently Monday through Friday,
however individuals can check out items on their own when the library is not
staffed. Find the library’s web page by going to www.Munsonhealthcare.org,
then click on the “Health Info” tab, then click on the Caregiver Resource
Center & Library. We
participate in an inter-library loan program with Munson’s Community Health
Library in Traverse City, so community members can access even more resources
without having to drive to Traverse City. Health
professionals associated with Munson or their affiliate hospitals, have access
to an inter-library loan program at both Munson’s Community Health Library and
Health Sciences Library located in Traverse City. The Caregiver Resource Center & Library is located
next to the main lobby at Paul Oliver Memorial Hospital, 224
Park Ave., Frankfort.

CITIZENS
FOR BETTER CARE

Citizens
for Better Care
is a statewide, non-profit, consumer
advocacy organization dedicated to improving the quality of care and quality of
life experienced by residents who reside in long term care facilities, adult
foster care homes, and homes for the aged.
It is the local long term care ombudsman program for Benzie County.
Citizens for Better Care provides advocacy for residents living in
long-term care facilities, adult foster care homes and homes for the aged,
community education and consumer awareness, and long term care information and
resources. All services are free
and confidential. Citizens for
Better Care is located at 1609 Park Drive, Traverse City.

COMPANIONSHIP PROGRAMS
·
The
Foster Grandparent Program utilizes
senior volunteers who are 60+ and on a limited income.
The volunteer receives a tax-free stipend of
$2.65 per hour, mileage at .31per mile, and $3.00 toward lunch.
Utilizes seniors in-group settings with children, such as schools,
daycares, juvenile detention centers, Head Start, etc.
The seniors work with children one on one or in groups at these settings
but do not work with them outside these sites or in their homes. Seniors are encouraged to become a foster grandparent.
The Foster
Grandparent Program is sponsored by Catholic Human Services and
is located in Traverse City.

Senior Companion Program
utilizes senior volunteers who are 60+ and on a limited income.
The volunteer receives a tax-free stipend of
$2.65 per hour, mileage at .31per mile, and $3.00 toward lunch.
Senior Companions are paired with other seniors who live in their homes,
nursing homes, Adult Foster Care or Assisted Living Centers and do a variety of
activities with their matched senior. Activities
depend on their needs and wants, such as taking to them to appointments or the
grocery store, going for rides, eating out, playing games or cards.
We are always interested in having more volunteers as well as more
referrals. The Senior
Companion Program
is sponsored by Catholic Human Services and is located in Traverse City.

COUNSELING SERVICES

Individual,
Family Counseling and Case Management for the Homebound
This service is provided to older adults who live in their homes, adult foster
care homes, assisted living facilities, or nursing homes in Benzie County.
Home visits are limited to those individuals who cannot come into the
office for treatment. This program is sponsored by Manistee-Benzie Community Mental Health, located at 6051 Frankfort Hwy, Suite 200, Benzonia.

Counseling
Services for Medicare/Medicaid Assistance Program This is a free counseling service to those aged 65
and older, and those who are Medicare beneficiaries due to disability and their
families. Information and
assistance in the areas of: Understanding doctor and hospital bills and Medicare
Summary Notices; Help with Medicare insurance enrollment, claims, and appeals;
Assistance in filing Medicaid applications, appeals, and post-enrollment
problems; Review of Medicare supplemental insurance needs and policies (Medigap
Insurance); Exploration of long-term care insurance needs and policies. The
Benzie County Council of Aging is located at 10542 Main, Honor. The Area of Aging of Northwest Michigan is located in

Benzie Area Christian Neighbors (BACN) Staffed by volunteers from county churches.
One coordinator who has the know‑how to arrange contacts with FIA
and other agencies is available to provide the public with vouchers for rent,
heating oil, used clothing, food, furniture, etc.
The coordinator is present during hours of operation:
Monday and Wednesday from 9 a.m.‑3 p.m., Friday 9 a.m.‑12

Community
Food Pantry Program (Benzie Food Partners) Set
up to operate a food pantry for low income Benzie County residents through a
volunteer network of donors and growers, to assist in meeting nutritional needs
of county residents. The pantry is designed to be "user
friendly" and centrally located in Benzie County to serve Benzie County
residents. The food pantry is open the 1st and 3rd Thursday each month.

Commodity
Food Programs
These
are USDA
programs that help clients with their monthly food needs by providing a box of
food. The Commodity Supplemental
Food Program is given monthly to clients over 60 years of age and 130% of the
poverty level. The Emergency Food
Assistance Program is given quarterly and has an income limit of 160% poverty
for seniors. Commodity Food
Programs are provided by Northwest Michigan Human Services and is located in
Traverse City.

Congregate
Meal Site Program There is a suggested donation for lunch.
Call to find out dates and times for special community luncheons with
speakers; recreational programs such as bingo, cards, bus trips to Traverse
City for lunch, shopping, movies, water aerobics.
New programs are being developed all the time. Meal reservations
are needed.
The Gathering Place
Benzie Senior Center is open from 8:00 a.m. to 3:30
p.m. Monday-Friday with lunch served

This program
delivers meals to homebound seniors, and provides breakfast, hot lunch, a brown
bag style meal for dinner, and frozen meals for weekends and holidays.

are usually offered during the fall and winter months, although pneumonia can be
given any time of year. Shots are
provided by a registered nurse and are free to those with Medicare Part B,
otherwise there is a fee for this service.

HOME HEALTH CARE SERVICES

Benzie
Community Services/Home Health Care Agency This program can provide
basic care only, including personal care and taking vitals. The service provides
a nursing assessment and follow up on clients when needed. A nurses aide will
visit once or twice a week depending on the need, help with vital signs, ostomy
care, bathing, dressing, grooming, nail care, assistance with ambulation and
exercises. Transportation services
are not provided at this time. Hours
of operation are Mon 12:30 p.m.- 4:30 p.m., and Tue-Fri 8 a.m.-12 noon.

Independence Agency Assistance with Activities of Daily Living Program
This service is for adults who are Medicaid eligible, either through straight
Medicaid or a Medicaid spend down. They
work closely with the Area Agency on Aging in regard to Medicaid eligibly. A
medical needs form has to be filled out by the primary physician.

In-Home Helpers List This is
a list of persons willing to be hired for household chores including
housekeeping, meal preparation, laundry, shopping, driving to the doctor’s
office, personal care, and respite care. Hours
of operation are Mon 12:30 p.m.- 4:30 p.m., and Tue-Fri 8 a.m.-12 noon.

In-Home Health Care Services This
service provides home health care including assessment, personal care, foot
care, light housekeeping, chores, laundry, medication preparation and
management, respite care (companionship), and transportation in Grand Traverse
and surrounding areas. Transportation
is available for appointments, errands, etc. and if a car is needed then mileage
is charged, subject to hourly rate. If
the clients have their own car then no mileage is charged.
Certified Nursing Aids and Registered Nurses are available.
R.N.’s provide medication management, foot care, blood draws, and
weekly assessments. 2 to 24 hour shifts are available.

and management, help with shopping, errands, transportation and other household

HOSPICE
AND PALLIATIVE CARE

Hospice
of Michigan
Hospice of Michigan is dedicated to providing care
and comfort to people in the final months of life, and support for their
families. Members of the hospice
care team help the patient, family and friends prepare for the end of life.
Every patient’s care is managed by an interdisciplinary team that
includes, physicians, nurses, social workers, counselors, aides and volunteers.
Hospice of Michigan’s grief support staff continues to offer emotional
support to family and friends after the death by providing a variety of
services. Hospice of Michigan offers grief support services to clients in
the community free of charge.

Munson
Hospice and Palliative Care: Offers
help and support to the dying patient and their loved ones, with treatment being
palliative rather than curative. Hospice
can be provided in a variety of settings including home, nursing home, assisted
living, or adult foster care. Hospice
services are available to persons having any terminal illness with a medical
prognosis of a life expectancy of six months or less if the illness runs it’s
normal course. Specific services
include nursing care, bathing and personal care, counseling services, pain and
symptom control, children’s art therapy, respite, grief support, dietary
counseling, physical therapy, and volunteer assistance.

Munson Hospice House:
Hospice
House is designed for individuals and family members needing end of life care
who require a level of assistance not available in their own home or other
settings. To be eligible patients
must be in a hospice program and consent to palliative treatment of care. The Munson Hospice House is located next to Munson
Medical Center at 450 Brook Street, Traverse City.

LONG

Paul
Oliver Memorial Hospital Long Term Care Unit
Paul Oliver
Memorial Hospital’s Long Term Care Unit is a 40-bed, nonprofit, skilled
nursing facility. Each resident has
immediate access to physical therapy, cardiac rehabilitation, acute care and
emergency care services. The resident’s are cared for by an interdisciplinary
team that provides individualized care to each resident.
It is a licensed Eden facility with the goal to promote and improve the well being of the residents by creating a
community that is healthy, safe, fun and rehabilitative. When arriving on the unit, everyone can enjoy a comfortable
home environment including the company of birds, plants, fish, and cats.
The unit has an intergenerational program to stimulate and encourage
spontaneity among residents, while the on-site Caregiver Resource Center &
Library is available to offer reliable general health and caregiver related
information to family members as well as the general community.

Maples, Benzie County Medical Care Facility
The Maples is a
62 bed, nonprofit, skilled nursing facility.
The Maples is county owned but governed by the Benzie County Family
Independence Agency. The Maples
provides a complete range of services from sub-acute rehabilitation to extended
stay. Our skilled, professional
team of therapists are committed to providing individualized programs focusing
on returning the resident to their defined level of optimal, functional,
productive, and satisfying lifestyle. Our
team of professionals provide individualized programs and plans of care to
promote the highest level of independence.
We are an Eden Alternative home who uses the Eden philosophy to enhance
environmental stimulus with the unconditional love of its cats, birds and
visiting dogs. The Maples is
located at 210 Maple Ave., Frankfort.

EDUCATION PROGRAMS

Family
Nutrition Program
Clients must be a Benzie County resident and meet income requirements.
They engage seniors with other seniors in nutritional settings; work to
improve nutritional information; provide information related to different areas
including food safety, how to read food labels, cost issues, recipes for using
commodity foods; and do both group and individual work. Services are free if
persons meet income guidelines. The
Family Nutrition Program is located at the Michigan State University
Extension Office,
P.O. Box
349, Beulah.

Outpatient
Diabetes Education
is a comprehensive diabetes management program for you and your family
including nutritional counseling and food management by a registered dietitian
and medication counseling and education by a registered nurse. For
your convenience, this program has been made available to you at Paul Oliver
Memorial Hospital in Frankfort and at Munson Medical Center in Traverse City.
When calling to schedule an Outpatient Diabetes Education appointment, let them know which location you
prefer.

Nutritional Counseling
is serviced by a registered dietitian who provides individualized
nutrition education on special diets, weight management, and wellness taking
into account personal lifestyle and food preferences. Outpatient

Frankfort.

PHARMACY AND PRESCRIPTION
MEDICATION PROGRAMS

Community Pharmacy Program Programs
to assist low‑income older adults, who have no prescription coverage, are
unqualified for other programs; meet income‑based eligibility criteria,
and who need medications for CHRONIC medical conditions. Program assists them in obtaining their medications through
the Patient Assistance Programs of major pharmaceutical companies.

· Northern Prescription Discount Plan This plan offers anyone without prescription insurance, a discount prescription card that is honored at any pharmacy in Benzie, Leelanau or Grand Traverse County.
